Residential Coordinator

Summary of position/Objective:

Provide support in the Residential Department in the day to day activities, including performing monthly home inspections, attending SP and IDT meetings and ensuring paperwork is up to date and complete.


Essential Duties and Responsibilities:

  • Maintain a work environment that embraces and fosters a culture of safety
  • Perform physical Home Assessments of To The Rescue’s various residential settings for compliance with regulations and with attention to resident safety.  This includes initial assessments prior to placing a resident, as well as ongoing Monitorings of PCAs, Host Homes and Apartment settings
  • Ensure completion of Annual paperwork as required, to include but not limited to:  Health and Safety Assessments; Consumer Life Reviews; HRC Reviews; and Consumer Assessments
  • Complete, organize, and maintain all consumer files.  This includes PNotes, ISSPs, BISSPs, MARs and the tracking of such items where required
  • Attend  IDT, SP and HRC Meetings as needed
  • Ensure consumer medical needs and records are kept up to date and maintained in accordance with the rules and regulations set forth by the State of Colorado.  Coordinate as needed with RN
  • Ensure MARs are accurate and completed appropriately for each consumer; ensure MRBs are filled appropriately in each location, if applicable
  • Ensure that Incident Reports and Risk Management Reports are completed same day and forwarded to the Director of IDD Services within 24 hours
  • Work with Residential Manager and Host Home Providers to ensure monthly Fire Drills are performed and required paperwork is turned in each month
  • Communicate with parents, guardians and Host Home Providers in a proactive manner
  • Ensure all required paperwork and tracking is completed in a timely and accurate manner.  Work with all caregivers and Host Home Providers to ensure all paperwork is on file
  • Ensure that the consumer experience is of the highest quality and that the goals of the Service Plan are met
  • Adhere to all regulations and rules pertaining to working with IDD consumers
  • Communicate issues and concerns in a timely manner to the Residential Manager
  • All other duties as assigned

  • Experience in working with individuals with Intellectual and Developmental Disabilities
  • Ability to Multi-task throughout the day
  • Ability to communicate with employees, independent contractors as well as consumers
  • Have patience, compassion and empathy
  • High level of organization and attention to detail
  • Ability to work in a fast-paced environment
  • Ability to work as member of a team
  • Adequate computer skills including experience working with Microsoft Office, email and internet


Expectations:

  • Work flexible schedule including meeting on-call requirements
  • Keep Residential Manager updated on schedule changes as they occur
  • Comply with proper safety policies and procedures (including all laws)
  • Provide an exceptional level of consumer service to all Residential consumers
  • Maintain a valid driver’s license and good driving record
  • Follow all To The Rescue policies and procedures

  • Exhibit professionalism in everything you do


Certificates/Licenses/Registration:

The employee shall possess a valid driver’s license and must be eligible for the agency’s vehicle insurance and provide proof of private vehicle coverage. 


Supervisory Responsibility:

This role will not directly supervise employees.


Work Hours:

Hours are varied depending on the needs of the organization. This may include, but is not limited to days, nights, weekends, and holidays. 


Work Environment:

The work environment is consistent with similar office environment settings.

Travel:

Frequent travel within a 50-mile radius is expected for this position. Occasionally travel beyond a 25-mile radius may be required, including out of state travel.
