Human Service Coordinator

Summary of position/Objective:

The Human Service Coordinator provides services to individuals with developmental disabilities, brain injuries and/or mental illness who live independently in community settings.

The Human Service Coordinator will manage all aspects related to the ongoing implementation of the Human Service Program.  This includes but is not limited to the quality assurance functions daily on all support paperwork and documentation to remain compliant with standards set by Iowa Code, the Social Security Administration, Managed Care Organizations, Iowa Medicaid Enterprises, and To The Rescue Policies.


Essential Duties and Responsibilities:

  • Adheres to and upholds To The Rescue’s values and policies
  • Works with Human Services Supervisors to ensure employee schedules meet member and agency needs
  • Works with Human Services Supervisors to ensure accuracy of time cards and approval based on agency deadlines
  • Responsible to work with Human Services Supervisors on employee reviews, disciplinary action, and termination
  • Monitor and control expenses to ensure program efficiency
  • Follow and adhere to all paperwork guidelines
  • Process member referrals, attends annual meetings, and completes all paperwork as needed
  • Work directly with Quality Assurance to develop and to keep all paperwork up to date and compliant with all standards
  • Responsible for attending member meetings and trainings that include, but are not limited to county providers meetings, IACP, and train the trainer
  • Provide guidance to Human Service Supervisor’s for initial and ongoing training of employees working within the Human Services department to remain compliant with program standards
  • Support Human Service Supervisors to effectively manage the day to day activities of each client assigned to their caseload
  • Develop and maintain positive professional relationships with all individuals included in each member’s interdisciplinary team, internal, and external stakeholders
  • Supports the Human Service Supervisor to ensure shift coverage
  • Ensure the health and safety of members by monitoring service needs
  • Reports maintenance concerns timely to Operations for approval
  • Provide updates to internal and external stakeholders timely
  • Always represents To The Rescue in a positive and professional manner to all members, co-workers, interdisciplinary teams, and the general public
  • Performs additional duties as assigned
  • Follow and adheres to the On-call policy and procedure within the Human Services Department

  • Must have excellent communication skills and the ability to communicate effectively with staff and provide instruction and direction to employees on maintaining compliance with policies, rules and procedures
  • Proficient with Microsoft Office that includes Word, Excel, Outlook and the ability to learn computer programs for scheduling and timekeeping
  • Ability to respond well under pressure


Certificates/Licenses/Registration:

The employee shall possess a valid driver’s license and must be eligible for the agency’s vehicle insurance and provide proof of private vehicle coverage.


Supervisory Responsibility:

Directly supervises Human Services Supervisors and support Direct Support Professional position. 


Work Hours:

Hours are varied depending on the needs of the organization. This may include, but is not limited to days, nights, weekends, and holidays. This position participates in an on-call rotation.


Work Environment:

The work environment is variable, anywhere from office setting to a hospital/clinic to out in the community.

While performing the duties of this job, the employee is regularly required to hear and speak. The employee is frequently required to sit, stand, walk, reach, and occasionally lift up to 80 pounds.


Travel:

Frequent travel within a 25-mile radius is expected for this position. Occasionally travel beyond a 25-mile radius may be required, including out of state travel.
