Registered Nurse Homecare Case Manager

To The Rescue is a growing
Home Health Care company that is seeking a Full-Time Registered Nurse (RN) Case
Manager to coordinate and provide compassionate skilled nursing care on an
intermittent basis for adult patients in their homes. This position is Monday-Friday without on-call responsibilities.
Summary of position/Objective:
The Case Managing Nurse is
part of a healthcare team (patient, family, nurse, physician, therapist and
others as applicable) that work together to help our patients reach and
maintain their highest level of independence with the goal to remain safe at home.
Along with other duties, the nurse will perform direct care skilled visits
according to the physician plan of care orders, complete comprehensive patient
assessments, develop a plan of care in coordination with the healthcare team
and communicate effectively to meet the patient needs. The nurse will insure
that services are delivered according to licensing guidelines, professional
medical standards and agency policies and procedures.
Essential Duties and
services in accordance with school health plan.
evaluates the patient's nursing needs.
school health plan and informs supervisor of any needed necessary revisions.
those services requiring substantial specialized nursing skills.
appropriate preventive and rehabilitative nursing procedures.
required documents/ notes for patient each day and submits to the school
district as required.
school district nurse and patients' parent(s) of changes in the patient’s
condition and needs.
in in-services programs, supervising and teaching other nursing personnel.
and adheres to established Agency policies and procedures.
patient care as needed.
confidentiality, safeguards all patient related information.
meetings and patient care conferences as scheduled.
documentation and paperwork in a timely manner per school district/ agency
reports to Clinical Manager any patient incidents/ variances or complaints.
competent performance of technical skills according to established procedures.
and adheres to established policies/procedures.
Agency standards and consistently interprets and accurately performs all
acceptable attendance status per Agency policy.
incomplete work assignments to Clinical Manager.
sound judgement, critical thinking and decision making.
current CPR certification.
other duties as assigned.
an approved school of professional nursing and currently licensed in the
state(s) in which practicing.
One (1) year
nursing experience preferred.

Graduate of an approved school of professional
nursing and currently licensed in the state(s) in which practicing.
One (1) year nursing experience preferred.
