LPN BB Assisted Living

Purpose- 

The Licensed Nurse assists the Assisted Living Director in directing the Assisted Living Staff Nursing Staff.  Responsibilities include direct supervision of the nursing staff as they provide care to the Assisted Living residents.  In addition, and when applicable and necessary the Licensed Nurse provides or directs care to residents of the greater Mayflower Community Campus, including residents of  Independent Living, Assisted Living and Residential Care.  The Licensed Nurse assures that nursing services meet and exceed Mayflower Community and government regulatory standards in accordance with Best Practices set forth by the Iowa Board of Nursing and the medical community and that optimal standards of quality of care and quality of life are met.  The Licensed Nurse follows OSHA regulations to create and maintain an environment that protects the health and safety of the staff as well as the residents.  


Essential Functions- 

  • Provides nursing care that consistently meets/exceeds professional standards in compliance with Federal and State regulatory guidelines including health and safety guidelines set forth by OSHA.
  • Assigns direct care staff in the performance of their duties, ensuring that care rendered by direct care staff meets/exceeds health care standards and is in compliance with State of Iowa and Federal Regulatory standards, including OSHA. 
  • When needed the Licensed Nurse teaches, redirects, assists direct care workers to improve/refine their skills and the quality of their care. 
  • Implements Nursing Department and Mayflower Community Policies and Procedures
  • Maintains communication with families, direct care workers, physicians and their designees, other service providers and agencies, and other departments within the organization.  Provides clear, concise information and feedback.   
  • Notifies family members of any change of condition or any new orders.     
  • Utilizes facility-provided radio communication system to communicate with direct care staff. 
  • Maintains each resident’s right to privacy and confidentiality by strict adherence to HIPAA standards directing the dissemination of medical and personal information. 
  • Is proficient in facility-specific computer software, having the ability to document and review medication administration, narrative notes, physician orders, and other components of the e-chart
  • When applicable is able to utilize the computer-generated medical record to transmit necessary information to other providers as requested and in strict adherence with HIPAA standards.
  • Maintains strict accounting of any narcotics or controlled substances by signing out controlled substances promptly, participating in counting narcotics/controlled substances each shift, and reporting any deviations in controlled drug count to the DON immediately. 
  • Prepares, administers and accurately documents oral, topical, inhaled  and  injectable medications and treatments to residents with consistent accuracy and within a specified time frame. 
  • Documents reason and response when administering PRN medication.     
  • Assists in maintaining a complete medical record for each resident, including the Resident’s Care Plan and makes adjustments and modifications to the Care Plan as interventions are added and changed, and the resident’s condition changes. 
  • Communicates changes in condition to MDS/Care Plan Coordinator. 
  • Modifies the Care Plan as new orders are received.
  • Conducts thorough and accurate resident assessments.
  • Documents findings in a clear and concise manner.  (Admission assessments are to be conducted only by Registered Nurses)
  • Identifies skin impairments. 
  • Reports and documents as appropriate and assists in weekly monitoring. 
  • Is alert for changes in resident condition and reports such changes promptly to physician and to family members.
  • Identifies serious or life-threatening situations with residents. 
  • Acts quickly and decisively to provide emergency care as necessary, to communicate with physicians and family, and to facilitate prompt transfer to local hospital  for emergency services if indicated and as order by physician. 
  • Is aware of and considers each resident’s Advance Directives, CPR status, etc. in determining best course of action and communicates Advance Directives and CPR status to medical and emergency personnel. 
  • Prepares and sends Transfer Packet with the resident including copies of IHOP (Advance Directives) and  medication administration record . Contacts ER nurse and gives phone report regarding the resident they will be receiving. 
  • Has the ability to think quickly and respond appropriately in the event of disaster, threatening weather, fire or other emergencies that may occur in the facility. 
  • Is able to handle emergency equipment. Directs staff during emergency. 
  • Promptly alerts Facilities Management and other emergency personnel as indicated.  
  • Helps protect residents and staff during an emergency.
  • Recognizes and respects the emotional, social, cultural and spiritual needs of each resident. 
  • Is sensitive to a resident’s feelings and needs. 
  • Provides reassurance, comforts residents. 
  • Provides compassionate care for the dying resident. 
  • Is sensitive and respectful of each resident’s particular wishes and preferences associated with end-of- life issues. 
  • Provides support and caring to family members.   
  • Utilizes knowledge of analgesics and other medications given to dying residents. 
  • Administers pain medications appropriately and in accordance with accepted standards.
  • Follows facilities Comfort Care standard to assure that all is being done to maintain the resident’ s physical, emotional, psychosocial and spiritual comfort. 
  • Involves clergy in accordance with resident and/or family’s desires.   
  • Utilizes outside resources such as Hospice when applicable and functions in cooperation with Hospice when they are involved.


General Functions- 

  • Ensure strict confidentiality of residents’ personal and medical information in accordance with HIPAA law. 
  • Abide by all Federal, State, local and facility rules and regulations pertaining to the Nursing Services Department including all OSHA regulations, Education requirements and meetings/in-services as scheduled.
  • Arrive for work promptly when scheduled.
  • Accept work assignments and complete them in an effective and professional manner.
  • Assign direct care staff appropriately. 
  • Request feedback at the end of the shift from direct care personnel. 
  • Conduct yourself in a manner that fosters cooperation and teamwork among coworkers. 
  • Be supportive and respectful of all team members. 
  • Provide assistance to them as needed. 
  • Conduct yourself in a manner that reflects that you are a professional nurse. 
  • Cooperate with residents, staff, other departments, administration, physicians, consultants, other service providers, families, and senior management staff.
  • Have respect for a resident’s personal property.
  • Manage supplies and equipment to avoid unnecessary waste and expense.

Qualifications- 

  • Current valid nursing license
  • Current certification in CPR
  • Working knowledge of Federal, State and Local long-term care regulations
  • Literacy in the English language
  • Ability to function with autonomy
  • Ability to follow written and oral directions and to give written, oral, and/or recorded reports.
  • Ability to supervise, direct, teach, train CNAs and CMAs and to delegate tasks, to clarify, redirect as needed.
  • Possession of critical thinking, problem solving and trouble-shooting skills
  • Possession of interpersonal and communication skills
  • Understanding of environmental/occupational exposure risks involved:  refer  to Hazard Risk Analysis. Understanding of OSHA requirements, use of SDS  information, use of PPE (personal protective equipment).   
  • Understanding of your potential for exposure BBP (blood borne pathogens) and Hazard Materials. 
  • Participation in facility In-service Education through in-house and online education sessions.
