Reports To: Licensed Nurse, Director of Nursing, Assistant Director of Nursing


Purpose- 

The CNA, under direct supervision of the Charge Nurse (Licensed Nurse in Charge) is responsible for providing direct care to residents of Mayflower Community. CNAs are aware of State, Federal and facility policies and regulations and provide care that meets/exceeds accepted standards.


Essential Functions- 

  • Under the supervision and direction of the Licensed Nurse, the CNA provides direct care to residents of Mayflower Community Health Center or Assisted Living.
  • The CNA assists residents with activities of daily living (ADLs) ensuring cleanliness, grooming, nourishment, rest, activity and elimination in a manner conducive to their comfort and safety.
  • Responds promptly to residents' concerns and addresses them appropriately, reporting to the Nurse in Charge when indicated.
  • Answers call lights promptly.
  • Is alert for changes in a resident's condition, including physical changes, pain, mood, behavior, functional ability, etc.
  • Promptly reports changes to the Licensed Nurse.
  • Preserved each resident's dignity and respect at all times.
  • Maintains each resident's right to privacy and confidentially by strict adherence to HIPAA standards and safeguarding each resident's medical and personal information.
  • Assists in providing a safe, comfortable and home-like environment for all residents.
  • Has knowledge of and follows each resident's Plan of Care.
  • Has input into the care-planning process by offering suggestions to the care plan team.
  • Communicates effectively with co-workers, supervisory staff, and other personnel within the Nursing Services department and with members of other departments, providing clear, concise, accurate information and feedback.
  • Use radio system to communicate with nurses and coworkers.
  • Documents that care tasks are completed and enters pertinent resident information on Mayflower's computer software.
  • May perform certain nursing procedures not requiring professional evaluation.
  • Recognizes and respects the individual emotional, social and cultural needs of reach resident.
  • Transports residents as needed. 


General Functions-

  • Ensures confidentiality of resident and facility information.
  • Follows Mayflower Community's Social Media Policy.
  • When on duty uses personal cell phone only as allowed by with Mayflower cell phone policy.
  • Abides by all Federal, State, local and facility rules and regulations pertaining to the Nursing Services Department.
  • Arrives at work on time and accepts work assignments.
  • Prioritizes and organizes tasks and completed assignments in an effective, professional and timely manner.
  • Cooperates with co-workers within the Nursing Department and with workers in other departments.
  • Follows directions from Nursing Supervisors and the Licensed Nurses in charge.
  • Provides feedback regarding the status of assignments and any pertinent information about residents.
  • Demonstrates respect for each resident's personal property and personal space, i.e. apartment or room.
  • Manage supplies and equipment properly in order to avoid unnecessary waste and expense. 
  • Functions as part of a team.
  • Conducts self in a manner that fosters teamwork among coworkers.
  • Is supportive of coworkers and provides assistance to the team members as needed.
  • Follows safe practices as defined by Mayflower policies and procedures and in accordance with OSHA guidelines.
  • Helps to maintain a safe and healthy environment within the facility.
  • Demonstrates proficiency in using special equipment (such as mechanical lifts, transfer devices, etc.).
  • Follows facility policies and manufactures guidelines for use of special equipment in such a way that prevents injury to staff or residents.
  • Remains current with disaster preparedness training.
  • Has the ability to think quickly, respond calmly in the event of threatening weather, fire or other emergencies.
  • Helps to keep residents, self and other staff safe.
  • Is able to perform the Heimlich Maneuver to assist the choking resident.
  • Is able to assist the licensed nurse in emergencies. Is aware of location and operation of emergency equipment (back boards, suction machine, CPR face masks, oxygen concentrators, etc.).
  • Performs other duties as assigned by supervisor.

Qualifications- 

  • Current valid listing on Iowa Direct Caregivers Registry
  • Is a Mandatory Reporter with current training and certification regarding Dependent Adult Abuse.
  • Familiarity with Federal, State, and Local long term care regulations, including OSHA safety standards.
  • Literacy in English
  • Ability to function with autonomy.
  • Ability to follow oral and written directions and assignments and give oral and written report.
  • Possession of interpersonal and communication skills.
  • Possession of problem-solving and trouble-shooting skills.
  • Ability to perform various sensory/ cognitive and behavior activities.
  • Ability to work well with others.
  • Participation in Mayflower Community in-service education through in-house and online education sessions.
